This post presents a real case from Seoul The Nature Dental Clinic in Myeongil-dong, Gangdong-gu, Seoul, in which a gap between the upper front teeth was closed using the Bioclear method. A gap between the front teeth may look small, but because it is located right in the center of the smile, it often has a big impact on how people feel about their appearance. Many patients want to close the gap but are reluctant to have healthy teeth drilled down, and this case shows one conservative way to address that concern.

When the patient first visited, there was a clear gap between the two upper central incisors that extended up to the gum line. The space became wider toward the gum, and small triangular spaces were also visible near the gums between the neighboring teeth. This dark triangular space near the gum is called a black triangle. It often looks larger and darker than the actual gap, so it is usually the first thing noticed when a person smiles. Beyond appearance, such gaps can trap food and sometimes make it feel as though air escapes during speech. Above all, the patient wanted to fill the gap naturally without drilling the teeth.

Gaps between front teeth have several possible causes. The teeth may be small relative to the jaw or have a triangular shape that narrows toward the gum. The gums and supporting bone may have receded, leaving empty space between the teeth. Gum disease can cause teeth to drift apart, tongue-thrusting or lip habits can push the front teeth forward, and spaces can remain near the gums even after orthodontic treatment.

A black triangle appears when the gum tissue between the teeth, called the interdental papilla, does not completely fill the space between them. If gum disease is the cause, it should be treated first. In this case, the gums were stable, so treatment could proceed directly.

Bioclear is a resin technique that uses a clear, curved matrix shaped to match the natural contour of the tooth. After the matrix is placed, heated composite resin with improved flow is injected in a single step. This allows the gap and black triangle to be filled while preserving healthy tooth structure, with little or no drilling. Conventional resin bonding typically uses a flat band and builds up resin in several layers, which makes it difficult to recreate the convex shape of the tooth near the gum and can leave tiny gaps between layers.

Because Bioclear uses a matrix shaped like the tooth from the start, it can fill the space with a natural bulge all the way up to the gum, which is an advantage when reducing black triangles.

The treatment is carried out in four steps. First, biofilm on the tooth surface is thoroughly removed by blasting, which is essential for strong bonding. Second, a clear curved matrix that matches the shape of the tooth is placed between the teeth to design the contour up to the gum.

Third, heated resin is injected in one step to fill the space completely and then hardened with light. Finally, the shape is refined and polished to give a natural gloss and proper bite. Because healthy teeth are not drilled, the procedure can usually be done without anesthesia, and depending on the number of teeth and the extent of treatment, it is often completed in a single visit.

After treatment, the two central incisors met naturally, and the space was filled with a gentle bulge up to the gum, so the dark triangular space was no longer visible. The width and length of the two front teeth were balanced, and the resin shade was matched to the surrounding teeth so that the restoration blends in. Compared with before treatment, when the dark gap was the first thing noticed, the front teeth now form a single, even line that gives a brighter and neater impression. Results vary depending on the size of the gap and the condition of the gums.

Other options for closing gaps include laminate veneers and orthodontic treatment. Bioclear involves almost no tooth reduction and a short treatment time, but as a resin restoration it requires care to limit staining and wear over time. Laminates can change color and shape more dramatically and resist staining, though some reduction of the front surface may be needed. Orthodontics moves the teeth without drilling but takes longer and requires a retainer. If the gap is very large or a major change in color and shape is desired, laminates, crowns, or orthodontics may be more suitable, while Bioclear can be a good choice for smaller gaps when preserving tooth structure is the priority.

To help the result last, patients should clean between the teeth with floss or interdental brushes, avoid biting hard objects with the front teeth, rinse after dark-colored drinks, and attend regular check-ups for polishing. A further advantage of resin is that small chips or stains can often be repaired locally.
